Toolbox Talk and Pre-shift Briefing Template

Purpose. The standard record for a toolbox talk or pre-shift briefing on a boat, in the plant or at the distribution centre, showing the topic, hazards discussed, controls agreed and attendance.
1.How to use this template
Use this template for the daily pre-shift briefing in the plant and distribution centre, the pre-trip briefing on vessels, the morning farm briefing and any topic based toolbox talk. The leader completes the topic, hazards and controls, and every attendee signs. The completed record is filed at the site and summarised in the monthly safety report.
2.Choosing the topic
Topics must come from recent incidents and near misses in the WHS Incident and Hazard Register, seasonal hazards such as heat, cold and heavy weather, changes to procedures or equipment, and audit findings. The leader must check the register before every talk.
3.Running the talk
Keep the talk to ten to fifteen minutes. State the hazard, ask the crew what has gone wrong or nearly gone wrong, agree the controls, and confirm who does what today. Record questions raised and any hazard reported during the talk.
4.Follow up
Any hazard or action raised must be entered in the WHS Incident and Hazard Register with an owner and a date. The leader must report at the next talk on what was done.
5.Records
Toolbox talk records are evidence of consultation and training and must be kept for five years.
